These templates have been uploaded on this website for every Joint Health and Safety Committee to use. They appear as appendices in the JHSC Handbook 2007. These templates are available to standardize the submissions of each JHSC in accordance to the Occupational Health and Safety Act of Ontario.

Appendix A: Draft Memorandum Requesting Committee Membership

  • This is to be completed when the Responsible Line Manager informs all eligible academic and administrative employees of the requirement for a committee in the workplace, and request volunteers or nominations. [4.3, pg4, JHSC Handbook 2007]

Appendix B: Committee Membership Listing

  • Responsible Line Manager shall ensure that an up-to-date list of the names, work locations and telephone numbers of committee members is posted in a conspicuous place in the workplace where it is most likely to come to the attention of employees, such as on local health and safety bulletin boards. [4.9, pg5, JHSC Handbook 2007]

Appendix C: Draft Terms of Reference

  • The Terms of Reference specifies the membership, the method of selection, and the workplaces represented by the committee, as agreed upon by the Responsible Line Manager and the committee members. [6.1, pg5, JHSC Handbook 2007]

Appendix D: Sample Agenda for Committee Meeting

  • Committee co-chairs prepare the meeting agenda and distribute it to all members, the Responsible Line Manager, the Office of Environmental Health and Safety and the trade unions represented on the committee at least one week in advance of the date of all regularly scheduled meetings. . [7.7, pg8, JHSC Handbook 2007]

Appendix E: Meeting Minutes Template

  • Minutes contain the names of the members, specifying those who were present and absent, and indicating whether they were management (M) or worker (W) members. The minutes shall also contain the names of other persons in attendance, the details of all matters discussed, and the committee’s recommendations for corrective action. . Appendix F: Sample Schedule of Workplace Inspections

  • the committee must establish a schedule of inspections when it is not practical to inspect the entire workplace once a month, such as when the workplace is too large or complex. At least part of the workplace is inspected monthly and the entire workplace is inspected at least once a year. Workplace inspections are to be conducted in accordance with this schedule. Appendix H: Items to be posted on the JHSC bulletin board
